Important Notice for Current and Previous Employees of Columbia Vehicle Group RE: Form 1095-C

As of the 2024 Tax Year, IRS regulations have changed with regard to provision of 1095-C forms which relate to proof of health insurance options offered to employees.

Instead of automatically providing printed version of these documents, we are now informing you that you may request a copy of your statement. With our HRIS system through UKG, you actually may print out your own 1095-C statement if desired, or you may request that a copy be provided to you via the Human Resources office at your specific work location.

Please note: The 1095-C statement is not required when filing your income tax return. It simply is information that should be retained with your personal income tax records.
